Petition activity is increasing, and with it the need to submit them a legible and complete form.
You must provide your applicant with the new single-page petition form. Your Secretary has the new forms.
Please remember that your secretary is required to run a simple case search through the Wisconsin court system and needs your help to get the correct results.
1. Have the applicant print or type all information.
2. Verify applicant’s date of birth. (Month – Day – Year)
3. Request the applicant to provide full name, initials can be a problem.
4. Place of birth is city and state, not hospital. (How many St. Agonies Hospital are there?)
5. Be sure that all questions are answered completely.
6. Be sure that you can read all entries without effort.
Remember that the $75 petition fee must be submitted with the petition.
Are you a Veteran of the United Stated Military? If so and you wish to have your patriotic service recorded as part of your Masonic history, please provide your branch of service, date of enlistment (induction), date of discharge (separation), conflict and any additional comments you would like to have entered.
Remember – “You Are Part Of The Plan”, your dues for 2011were payable by December 31, 2010. If you did not receive your notice, did not or cannot make payment by that date, PLEASE contact me before August. I will work with you in a confidential manner.
